- a valid photo id (drivers license, state-issued ID card, passport book/card, goverment ID, military ID, etc)
- a check or money order for $110 made out to the US Department of State
- - standard processing time is 4-6 weeks, to expedite the processing time down to 2-3 weeks, you may pay an optional extra $60
- cash, check, or credit card to pay our facilities $35 processing fee
- appear in person in front of me with the above, issue your oath, sign and seal the form, and send it to the passport processing facilities
