It was said before but ya heroin is relatively middle of the pack. Nothing comes close to fentanyl and fentanyl abuse is becoming more common place. Fentanyl is only prescribed as a patch but we've gotten reports of people trying to extract the opiate by boiling the patch and the distilling to get pure fentanyl. IV fentanyl can Kill you if it's off by a few milligrams. So..ya.

Dilaudid we give out like candy. The reason people turn to heroin at all is because street heroin is cheaper than prescription Dilaudid or morphine, also the state tracks how often those prescriptions are filled so people can get turned down.

I thought morphine, no matter the grade, was necessarily less potent than heroin and that the letter was essentially a refined version of the former. No?

No they're chemically different. I lump morphine heroin and oxycodone together in terms of potency...someone earlier correctly said that oxycodone is the usual gateway drug. When people develop tolerance to those they come to the ED asking for dilaudid...specifically IV dilaudid. The high from that is one of the strongest. Fentanyl has about 100x the potency of dilaudid and we only use it during operations.

True story: we recently fired an anesthesiologist because bottles of IV fentanyl were somehow disappearing after his operations. All bottles of fentanyl are supposed to go back to the pharmacy after an operation and they discard it in some specific way.
