The whole movie is from his perspective, so to him it's all really happening and everyone else conforms to that reality as well. The most telling scene is the one with him and Ramona at the bar before the fight with Mae Whitman, where Ramona calls him out on his bullshit and he lies about Knives and then spins an unhinged story about what happened with Kim to hide the truth about how he treated her. Just shows the lengths he's willing to go to keep living in denial about who he really is.

I know it’s from his perspective, but the battles he has to have.. is all that just built up in his head? Does he not have to fight anyone? The movie comes off as him being forced into those fights. It comes off as a part of that universe, not a figment of his imagination.

Yeah I've always seen it as the fights with the ex's are actually fights he's having both internally and with Ramona. His jealousy/judgmental beliefs manifesting themselves as video game-style brawls because he's delusional and sees himself as the hero of his story.

But honestly I mean idk I'm clearly in the minority here so I could just be projecting my own beliefs onto the movie, this is just what I've always taken away from it. I've always thought of it and the books as being very much against the kind of toxic behavior Scott exhibits.
